Avantic does not contractually participate with all carriers listed and intends this list as a guide to help you select plans, which usually reimburse Avantic directly. Coverage may vary based on the patient’s specific health plan benefit. Patients are responsible for payment of non-covered services, deductibles, co-payment and co-insurances, if applies.
